Coordinate essential services to maximize recovery and independence for individuals with mental illness. Provide clinical consultation to staff and implement individualized, goal-oriented strategies to improve consumer quality of life.
Requires a Bachelor's degree in a human services field and a valid Michigan Driver's License. Professional licensure (LLBSW, LBSW, LLMSW, LMSW, LMFT) and prior mental health case management experience are preferred.
Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness. In a residential or clinical setting, provide clinical and program consultation to direct care staff. Ensure case management compliance with agency standards and the practice model. Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community. Implement strategies for obtaining services and supports that are goal-oriented and individualized.
