Develop and document continuing care plans to ensure patients receive ongoing behavioral and mental health support after discharge. Coordinate discharge appointments and act as a liaison between patients, counselors, and community resources.
Requires an Associate's degree in nursing or human services, with a Bachelor's degree in social work or human services preferred. Candidates should ideally have experience in clinical settings and discharge planning, along with CPR and de-escalation certifications.
Southwood Psychiatric Hospital offers comprehensive mental health treatment programs for children and adolescents ages 3-21 whose lives have been disrupted by mental illness. Our facility located in the South Hills of Pittsburgh, Pennsylvania, provides many levels of care. Currently we are looking to hire a Discharge Planner. please note: Southwood is not on the public transportation line. PURPOSE STATEMENT: Provide assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility.
Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred. One or more years’ experience in a clinical setting preferred. One or more years’ experience in discharge planning preferred. LICENSES/DESIGNATIONS/CERTIFICATIONS: If applicable, current licensure as a LPN, RN within the state where the facility provides services. CPR and de-escalation and restraint certification required (training available upon hire and offered by facility. First aid may be required based on state or facility requirements.
