Provides social work services including psychosocial assessments and supportive counseling to patients and families. Coordinates discharge plans and utilizes community resources to ensure continuity of care and enhance coping abilities.
Requires a Master's degree in Social Work or Counseling with appropriate licensure (LSW, MSW, or equivalent). Minimum of 1 year of experience in a medical or clinical setting is required.
Provides a wide array of social work services to patients and families, including performing psychosocial assessments, developing and coordinating discharge plans to ensure continuity of care, and providing supportive counseling to enhance coping and functioning ability. May be asked to resolve issues occuring on all shifts, 7 days a week. Reports to Director of Clinical Bereavement. Performs other duties as assigned.
