Case Manager Registered Nurse | Home Health

• Find your purpose with CentraCare by providing care and treating patients where they’re most comfortable – In their home.
• As a Home Health Case Manager Registered Nurse, you will manage a case load of approximately 20-25 patients and care for around 5-6 patients per day.
• This allows you to give each patient the personalized attention and quality care they deserve in a less stressful environment.
• Common duties include wound management, infusion therapy, assessments, education and training for patients on their new diagnosis, and more.
• We provide extensive training and ongoing RN office support while you’re making home visits to patients.
• This position will be working primarily in Big Lake, Becker, Princeton, Zimmerman, Elk River and surrounding areas.

Schedule

• Full-time | 72 hours every two weeks (10 shifts per two weeks)
• Days | Monday - Friday | 8:00 am - 4:30 pm
• Every 8th weekend | 1 holiday per year | Rotating on-call

Pay and Benefits

• Pay based on experience and degree
• Associate/Diploma in nursing starting at $38.21 per hour
• Bachelor’s in nursing starting at $39.16 per hour
• Master’s in nursing starting at $40.14 per hour
• Pay range: $38.21 - $57.31 per hour
• Milage reimbursement and extra pay for working weekend shifts
• $3,500 hiring bonus for eligible candidates

Qualifications

• Graduate from an accredited school of nursing required
• Current Minnesota Registered Nurse license required
• Current American Heart Association (AHA) BLS Healthcare Provider card required
• Reliable transportation required
• Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage meeting CentraCare’s standards required
• Minimum of one year of RN experience

Based out of our office in Marketplace Mall in Waite Park, the Home Health department primarily takes place in the communities we serve, in the homes of our patients. Our staff provides services not just for St. Cloud but our surrounding areas as well including Buffalo, Alexandria, Little Falls, and more. Teams are assigned to areas to minimize driving.
