The Discharge Specialist provides case management and formulates discharge plans for behavioral health patients to ensure a smooth transition to the next level of care. This includes facilitating weekly discharge planning groups and collaborating with the treatment team to update patient databases.
Candidates must have a high school diploma or GED and at least one year of relevant healthcare experience. A bachelor's degree in Social Work or a related field is preferred.
A day in the life of a Discharge Specialist at Hackensack Meridian Health includes: In conjunction with the treatment team, formulating discharge plans for patients on the first day of admission for the patient. Begins discharge planning process within 24 hours of assignment. Participates in the treatment planning process. Continuously updates ACT database. Facilitates discharge planning group on a weekly basis on assigned units. Demonstrates positive communication techniques in working with other members of the treatment team in order to enhance patient care. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications: Education,
Required: High School diploma, general equivalency diploma (GED), and/or GED equivalent programs. Minimum of 1 year relevant work experience in a healthcare setting. Excellent written and verbal communication skills. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms. Education,
