Serve as the primary point of contact for hospice patients and families, delivering skilled nursing care and individualized care plans in home or facility settings. Collaborate with an interdisciplinary team to provide physical, emotional, and spiritual support while managing symptoms and end-of-life processes.
Requires an active RN license and at least one year of experience in hospice, home health, ICU, oncology, or geriatrics. Must possess a valid driver's license, reliable transportation, and CPR certification.
