Provide direct personal care, hygiene assistance, and emotional support to terminally ill patients in various home and facility settings. Monitor patient conditions and maintain accurate documentation while participating in quality improvement activities.
Requires an active CNA license, current CPR certification, a valid driver's license with a reliable vehicle, and at least six months of direct patient care experience. Candidates must be comfortable working independently in home-based or hospice environments.
