$29 - $41 / HOUR
Provides case management, assessments, and direct counseling to clients within public health program areas. Coordinates support services and collaborates with interdisciplinary teams to develop and monitor individual plans of care.
Requires a Bachelor's degree in Social Work or a related field and a valid Michigan driver's license. Depending on the program, a Licensed Social Worker (LSW) credential and specific experience in care management or maternal/infant support may be required.
Ability to communicate effectively orally and written. Skilled in client assessment and coordination of services. Skilled in communicating well with senior citizens and their families Skilled in communication with service providers. Skilled in use of EMR and various Microsoft Office products such as Word and Excel.
