The Social Worker provides psychosocial assessments and counseling to hospice patients and their families to coordinate end-of-life care. They collaborate with an Interdisciplinary Group to develop care plans and assist with funeral and discharge planning.
Requires a degree in Social Work (BSW or MSW) or a related field with appropriate supervision and licensure. Preference is given to candidates with one year of experience in palliative or hospice care.
Licensed as required by the state where practicing. Current drivers’ license.
One (1) year of experience in palliative care or hospice preferred.
Working knowledge of community resources. Good interpersonal skills. Demonstrates ability to work with computers.
Reliable transportation and valid auto liability insurance.
