Overview: The Registered Nurse (RN) position is responsible for providing professional nursing care for clinic patients following established standards and practices. Assists providers and staff in coordinating patient care and acts as a patient education resource. Functions as primary liaison between patients and providers. Each position has specific requirements with unique duties and responsibilities. This position may require travel between clinic locations.

Day in the
Life: Accountable for the coordination of nursing care, including direct patient care, patient & family education and transitions of care. Effectively practice within the model as established by PRH, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. Plan and prioritize work for patient care plans and daily planning of clinic to maintain professional delivery of nursing care. Interview patients and care givers to assist in the patient care plan based on nursing assessment, utilizing evidenced based practices. Document patient care conversations, care provided and all other work as defined by organizational standards in the EMR. Coordinates care as defined by organizational standards for defined patient panel. Decision-Making based upon professional nursing judgment within legal scope of practice. Promotes and contributes to a positive work environment, serving others with professional and positive attitude. Responsible to train, demonstrate, or guide others in the performance of assigned work, includes, but not limited to, new staff, precepting medical assistants in training and nursing students. Accountable to stay current with the changes in professional nursing and hospital policy. Ensure completion of regulatory and departmental competencies within the timeframe set by your supervisor. Active participation in quality improvement projects throughout the organization. Maintain a clean and safe work area and take responsibility for departmental space. About You!
