  • Schedule: Sat and Sun, 8 hours each day in patient's home Are you a compassionate, resourceful Social Worker who believes that quality end-of-life care begins with empathy and support? What You’ll Do As a Hospice Social Worker, you’ll play a vital role in helping patients and families navigate the emotional, social, and practical challenges that come with serious illness and end-of-life care in a home setting.

  • Responsibilities: Conduct comprehensive psychosocial assessments and develop individualized care plans for hospice patients. Provide clinical counseling and emotional support to patients and families, addressing social, emotional, behavioral, and spiritual concerns. Educate patients and families about hospice services, end-of-life planning, advance directives, and community resources. Offer guidance on behavioral health issues and provide crisis intervention when necessary. Advocate for patient needs, ensuring their rights are respected and upheld within the healthcare system. Collaborate with the interdisciplinary hospice team to coordinate holistic care and ensure continuity of services. Maintain accurate, timely, and confidential documentation in compliance with HIPAA and organizational standards. Qualifications
