The LPN is responsible for creating, implementing, and managing personalized care plans for individual patients. Key duties include administering medications, monitoring patient reactions, and maintaining accurate clinical records.
Candidates must be a Licensed Practical Nurse (LPN) in the state of employment with a valid driver's license. First aid/CPR certification is required, and previous LPN experience is preferred.
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans Administer medications and monitor patients for side effects or reactions Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with the supervisor
Currently Licensed Practical Nurse (LPN) in state of employment Previous experience as a LPN is preferred Excellent interpersonal and client care skills Valid driver’s license First aid/CPR certification required
