The Intake Coordinator facilitates the admission and discharge processes for clients, including processing paperwork and insurance information. They are also responsible for conducting level of care assessments and coordinating aftercare goals for detox clients.
A high school diploma or equivalent is required, while an associate's or bachelor's degree in human services is preferred. Candidates must pass a criminal background check, drug screening, and state-mandated clearances.
$15-$17 Major Areas of Responsibility: Explain and sign admission paperwork, process, and copy driver’s license/insurance information while creating and starting client treatment files. Contact Probation/Parole/Employers as needed for clients and assist in obtaining letters for admission/discharge. Facilitate the admission and discharge process by communicating with all relevant departments. Maintain paperwork, notes, and application statuses as required for client stay/discharge needs per funding source requirements. Conduct level of care assessments on all clients. Complete aftercare goals with detox-only clients, set up aftercare, and finalize discharge summaries. Perform other duties as assigned.
