Establish and grow a new urgent care center by delivering efficient, patient-centered care for acute illnesses and minor injuries. Work as part of a clinical team to provide high-quality healthcare to underserved populations across the lifespan.
Must be a NYS licensed Family Nurse Practitioner or Physician Assistant with a valid DEA and BLS/CPR certification. Requires a minimum of 3 years of clinical experience in urgent care, emergency medicine, or primary care.
Description Cornerstone Family Healthcare is a patient-focused Federally Qualified Health Center located in the Hudson Valley Regions of upstate New York – just 70 miles north of New York City. We are currently searching for a Family Nurse Practitioner or Physician Assistant to help build and grow our brand-new Urgent Care in Middletown, NY.

The position offers an exciting opportunity to help establish a new urgent care while working in a supportive, mission-driven environment. The provider will work as part of a two-person clinical team, delivering efficient, patient-centered care for acute illnesses and minor injuries across the lifespan. While Cornerstone providers collaborate closely, this role is ideal for an experienced FNP or PA who is confident practicing independently, making sound clinical decisions in a same-day care environment.
