The Social Worker provides psychosocial care, assessments, and support to hospice patients and their families. They coordinate with an interdisciplinary team to maintain individualized care plans and facilitate referrals to community resources.
Requires a Bachelor of Social Work from a CSWE-accredited school and at least one year of social work experience. Candidates must hold or be willing to obtain licensure in both North Dakota and Minnesota.
The Social Worker is an integral team member in the psychosocial care to hospice patients/families and coordinates with the interdisciplinary team (IDT) that is consistent with the goals, definitions, philosophy, and purpose of HIA Hospice. All psychosocial services to hospice patients and families including assessment, support and coordination of services. This is a full-time, benefit eligible position and based out of our Fargo office and supporting our inpatient unit, Heather's House. Key Responsibilities Conducts hospice referrals and accurately completes admission paperwork/consents. Conducts hospice comprehensive assessments at the time of admission. Assess the patients and family’s social emotions and financial needs. Assesses the patient’s/family’s psychosocial needs on an ongoing basis while hospice care is being provided through comprehensive assessment updates. Maintains an individualized, up to date care plan that reflects the care being provided to patients/families. Provides social work visits/services to patient/family as determined in the plan of care and through the plan in documentation. Provides PCG and family with periodic updates as indicated in the plan of care Refers patient/family to appropriate community resources. Coordinate patient care with contracted nursing homes and hospitals. Provides accurate, efficient patient status reports to team members in appropriate meetings, and in periodic updates. Demonstrates preparedness for IDT meetings and provides IDT updates to a teammate prior to any planned vacations. Complies with state licensure, federal certification requirements, and other existing contract/agreements related to patient care. Adheres to guidelines for personal safety of patients and families, including regulatory compliance Teaches patient/family in areas related to caregiving and in preparation for death as reflected in documentation Provides grief support to families surrounding the time of death. Refers PCG and family to bereavement in an appropriate manner. Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.

QualificationsQualifications Education: Masters of Social Work from a school accredited by the Council on Social Work Education, preferred; (at minimum Bachelor of Social Work) Experience: Minimum of one year of social work experience; health care experience preferred Strong interpersonal communication and counseling skills; team player Commitment to concepts of palliative care and hospice Certifications, Licenses, Registrations Required Licensure in North Dakota and Minnesota (or willing to obtain dual licensure upon hire) Current drivers license and auto insurance
