Perform patient screenings, intakes, and assessments while documenting progress in Electronic Health Records. Coordinate in-clinic and external referrals for social services and specialty care to improve patient outcomes.
Requires a Bachelor's or Master's degree in a social service field or an Associate degree with equivalent experience, plus at least three years of experience in social services or public health. Must possess a valid driver's license, car insurance, and no sanctions from healthcare regulatory agencies.
