Provide comprehensive mobile treatment and medication management for adults with persistent mental health illnesses. Coordinate care with outside medical providers and maintain professional relationships with clients and community agencies.
Must possess an associate or bachelor's degree in nursing and a valid Maryland nursing license (RN or LPN). A valid driver's license and access to an insured vehicle are required.
Assertive Community Treatment (ACT) [https://www.sheppardpratt.org/care-finder/assertive-community-treatment-act/] programs provide comprehensive, mobile treatment services for adults with a persistent mental health illness. The program is for individuals who have not found success in traditional outpatient settings and who require individualized treatment services when and where it’s needed.
What to expect.
This is an opportunity to help your clients, who may struggle with accessing standard care, meet their goals and ensure they receive the treatment they need.
Work hours typically follow a Monday – Friday schedule. Must have availability for occasional evening and/or weekend work. You will have independence and flexibility to schedule your appointments and complete your work under the program director’s direct approval and supervision.
What you’ll get from us.
Pay range for this position for RN is $80,000.00 to $88,000.00.
Pay range for this position for LPN is $57,782.40 to $59,571.20.
