The Care Coordinator manages the whole health of members using an integrated case management model involving assessment, planning, and monitoring. They act as a liaison between members and community resources while maintaining clinical records in the Electronic Health Record.
Candidates need a Bachelor's degree in behavioral health/social work, or an Associate's with 2 years experience, or a High School diploma with 4 years experience. Mandatory requirements include a DPS Level I fingerprint clearance and a valid Arizona Driver's License.
