The Case Manager identifies and coordinates resources to meet the individualized needs of individuals experiencing homelessness with mental health or substance use challenges. They implement service plans, advocate for participants, and collaborate with clinical teams to support overall wellness and stability.
A Bachelor's degree in Human Services or a related field is preferred, along with a valid driver's license held for at least three years. Candidates should have at least two years of experience in behavioral health, addiction recovery, or homeless services.

FaSST/Connections CTI Resource Coordinators engage individuals experiencing homelessness with a history or present struggle with mental health challenges and/or substance use.
The Case Manager is responsible for identifying and coordinating resources to meet the individualized needs of program participants. This role ensures implementation of service and treatment plans, advocates for participants to access necessary services, and helps identify program needs and service gaps while recommending improvement strategies.
The Case Manager works collaboratively with program participants, clinical teams, and community partners to support recovery, stability, and overall wellness. All responsibilities are performed in alignment with APIS and RHD organizational values.
