What you’ll do
As a Case Manager at the Crisis Intervention Center, you’ll play a vital role in guiding individuals through some of the most challenging moments of their lives.
You’ll work closely with a multidisciplinary team to ensure clients receive the care, resources, and support they need both during their stay and after discharge.
Here's what your day-to-day will look like: Coordinate admissions, transfers, and discharge planning in collaboration with the Clinical Director and care team.
Serve as a key liaison between clients, families, legal reps, and community service providers.
Develop and implement person-centered treatment and discharge plans focused on recovery, stability, and long-term success.

Identify appropriate services or placements following discharge and ensure timely, effective referrals (housing, outpatient care, substance use treatment, etc.).
Advocate for clients’ rights and ensure their voice is heard throughout their care journey.
Maintain up-to-date and accurate documentation of all case coordination efforts in compliance with HIPAA and KDADS regulations.
Work to remove common barriers to discharge such as lack of housing, transportation, or insurance.
Support the development of client-centered tools like WRAP (Wellness Recovery Action Plans).
Complete all required training in trauma-informed care, de-escalation, crisis response, and more.

Requirements
Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college. Must be at least 21 years old. Experience in mental health, crisis intervention, or substance abuse treatment preferred. Ability to handle high-pressure situations and respond to emergencies. Must pass KBI and KDADS background checks. Must pass drug test. Maintain valid driver’s license and insurable driving record (no substance-related incidents within 5 years). Must meet all health screenings, TB testing, and physical clearance requirements.
