The Physical Therapist will travel within Pierce County to provide intermittent care in patients' homes, working under the direction of the Director of Clinical Services. Responsibilities include evaluating patients, consulting with physicians, and supervising assistants as needed.
Candidates must be graduates from a physical therapist curriculum approved by the American Physical Therapy Association and currently licensed in Washington State. Two years of experience as a physical therapist and home health experience are preferred.

Full Time Mon-Fri $50-$60.00 Hourly DOE
A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
Travel within Thurston County to provide care on an intermittent basis in patients' homes. Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services. Work in conjunction with all other home care personnel in patient care planning. Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned. Take initial patient history and make initial evaluation. Consult with physicians regarding change in treatment. Instruct patients and family in home programs and activities of daily living. Participates in Quality Assessment and Performance Improvement activities as assigned. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition. Appropriately utilizes ICD-10 codes.
Graduate from a physical therapist curriculum approved by the American Physical Therapy Association. Currently licensed as a physical therapist in the state of Washington. Two (2) years' experience as a physical therapist, preferred. Home Health experience, preferred. Detail oriented, self motivated and capable of efficient time management. Critical thinker capable of working collaboratively with a team. Valid WA State driver's license.
