Act as the primary point of contact for participants in Kentucky Waiver Programs to ensure access to necessary resources. Create and manage personalized care plans while maintaining strong relationships with families and service providers.
Requires a degree in Social Work, Human Services, or a related field, or a combination of education and experience. Preferred candidates have 4+ years of professional experience in KY Waiver Programs or healthcare.
4+ years of professional experience in KY Waiver Programs as a Case Manager, or in a Human Services, Healthcare, or related field is highly preferred.
You must possess at least one of the following qualifications: A Bachelor's degree in Social Work, Human Services, or a related field. A Bachelor's degree in any field, complemented by at least one year of Human Services experience. An Associate degree in a Behavioral or Social Science field, with two years of Human Services experience. A Kentucky-licensed RN with two or more years of experience in the field of aging or disabilities. A Master’s degree in a Health or Human Services field from an accredited college or university. Relevant fields of study include: Rehabilitation, Nursing, Counseling, Education, Gerontology, Human Services, and Sociology. E.O.E
