We are seeking a full-time Physical Therapist to provide care for patients in both inpatient and outpatient settings.
In this role, you will work with individuals experiencing a wide range of conditions at Emplify Health by Gundersen, Moundview Memorial Hospital & Clinics and the Extended Care Center in Friendship, WI.
What you will do/job specifics Full-time 40 hours/week 8 hour shifts, 7am-3:30pm and 8am-4:30pm Occasional coverage on weekends Work closely with patients to assess their physical and psychosocial needs.
Planning, development, and implementation of multidisciplinary plan of care Evaluation of patients' responses to care.
Maintain patient records including initial evaluations/assessments, progress notes, and discharge summaries.
