Facilitate and manage Child and Family Team meetings to ensure continuity of care for Wraparound clients. Coordinate the implementation of care plans and serve as the primary contact for families and service providers.
Requires a Bachelor's degree in Human Services, Social Work, or Psychology, or three years of experience with high-risk youth. Must have a valid driver's license, registered vehicle, and pass background checks.
Serving as the contact point for child(ren), families, service providers, and approving Wraparound services for assigned clients Convening the team and ensuring continuity of care and that all identified services are provided and ensure family-centered practice by the team Assist in quality assurance activities including: case record reviews for consistency and quality, in addition to maintaining and updating documentation Assists in intake meetings with families to identify strengths, underlying needs, and informal support Facilitates planning, coordination, and implementation of Child and Family Care Plans at Child and Family Team (CFT) meetings Assists family and team in developing comprehensive Plan of Care and safety crisis plans Other duties as needed
Bachelor of Art or Science Degree in Human Services, Social Work, or Psychology; OR three (3) years’ experience working with high risk children/youth in either child welfare, probation or mental health systems in Wraparound Reliable transportation, valid driver’s license for one year minimum, and access to a registered vehicle with proof of insurance Successful clearance of local and/or state background checks Why Join Us?
