The MAT Care Manager provides direct nursing and care management services to patients accessing MAT services and coordinates with their care team. They also conduct MAT intakes and work collaboratively with patients in psychiatric and substance abuse programs.
Candidates must have a current Vermont RN license, with LPN considered, and prior nursing experience is required. Experience in case management in an outpatient setting is preferred.

The MAT care manager cares for patients accessing MAT services at Community Health and also coordinates with their care team members and other supports to help a patient manage their substance use, medical conditions and co-occurring behavioral health, psychological and social determinants of health through the healthcare system. MAT Care managers work closely with the primary care providers to establish treatment plan goals.
