$37 - $39 / HOUR
Provide direct and indirect nursing care to participants to support their health and independence through assessments and individualized care plans. Coordinate care across clinic, home, and hospital settings while managing medications and treatments.
Must be a graduate of an accredited School of Nursing with a current Michigan RN license, CPR certification, and a valid driver's license. Preferred candidates have three or more years of nursing experience, including work with frail elderly populations.
As a Clinic RN at Thome PACE, you’ll provide direct and indirect nursing care that supports participants’ health, independence, and overall well-being. In this role, you’ll use your clinical judgment to assess participant needs, complete required nursing assessments, identify changes in condition, and coordinate follow-up care in collaboration with the care team.
You’ll help develop and update individualized care plans, administer treatments and procedures, manage medications, provide participant and family education, and support care across clinic, home, hospital, and nursing facility settings as needed. This role also includes timely documentation, tracking hospital admissions and discharges, supporting infection reporting, and participating in team meetings, huddles, and quality improvement efforts.

Skills, Knowledge & Expertise Required Qualifications Graduate of an accredited School of Nursing Current Registered Nurse license in the State of Michigan Current CPR certification or willingness to obtain Valid driver’s license and reliable transportation
Three or more years of nursing experience in a healthcare setting At least one year of experience working with frail elderly or older adult populations Strong assessment, critical thinking, and care coordination skills Ability to communicate effectively with participants, families, providers, and care team members Comfort working in both clinic and community-based settings as needed
