Acts as a liaison between community members and health or social service systems to reduce health disparities. Responsibilities include conducting outreach, providing health education, and advocating for improved access to care.
Requires a high school diploma and the ability to obtain a Community Health Worker certification within one year. Strong communication skills and basic computer proficiency are required, with bilingual skills preferred.
The Community Health Worker (CHW) is a trusted connection between the community and local health and social service systems. In this role, you’ll build meaningful relationships, help individuals navigate care, and empower community members to improve their health and well-being.
This position plays a key role in reducing health disparities by providing outreach, education, advocacy, and support. The CHW works closely with healthcare providers, public health programs, and community organizations to ensure individuals have access to the resources they need.
