Our physicians play a pivotal role in enhancing community health through accessible, prevention-oriented, and collaborative healthcare services.

Essential

Duties and Responsibilities

• Patient Care: Conduct comprehensive health assessments, including physical examinations, ordering diagnostics, and developing individualized treatment plans.
• Prevention and Treatment: Emphasize disease prevention, health maintenance, and holistic patient wellness across all age groups.
• Collaborative Practice: Work in a team-based care model with nurse practitioners, physician assistants, and allied health professionals, ensuring cohesive and integrated care.
• Community Focused: Engage in outreach, patient education, and preventive care initiatives to reduce health disparities within the community.
• Documentation: Accurately document all patient interactions, diagnoses, treatments, and follow-up care in an electronic health record (EHR) system.
• Quality Improvement: Participate in quality improvement initiatives, peer review, and evidence-based practice updates to continuously elevate patient care standards.

Qualifications

Required Qualifications

• Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited medical school.
• Licensure &
• Certification:
• Active Illinois medical license.
• Board certification or eligibility (to be completed within two years).
• Documentation of completed residency.
• Experience: Proven ability to manage both common and complex health conditions within a community health setting. Experience with underserved populations is highly desirable.
• Skills:
• Strong analytical, problem-solving, and interpersonal skills.
• Proficiency with electronic medical records (EMR) systems.
• Ability to work effectively within a multidisciplinary team.

Tapestry 360 Health offers a comprehensive benefits package including health insurance, dental insurance, retirement savings plans, paid time off, continuing education support and eligibility for the Public Service Loan Forgiveness (PSLF) program. Pay Transparency: The salary range for this position is $160,000 – $200,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
