The Case Manager creates personalized service plans and goals for children and youth while providing social skills and behavior management support. They advocate for clients by linking them to community resources and adjusting treatment plans based on progress assessments.
Candidates must hold a Bachelor's or Master's degree in a related field such as Psychology or Social Work and possess CPR/First Aid certification. A valid Arizona driver's license, auto insurance, and eligibility for fingerprint clearance are also required.
06/01/2026 About Your Role Our Child-Youth Case Manager works with children and youth in the community, engaging them in the creation of their own service plan and goals, providing social skills and behavior management support/education, and advocating/linking them to additional community supports. You will meet with your clients and families in their homes, in the community, and in the office, to support their progress and assess needs to adjust the treatment plan as necessary.

Preferred Qualifications: ● Experience in Case Management ● Experience in Behavioral Health ● Bilingual: English/Spanish
