This position focuses on delivering Community Habilitation services that empower and encourage personal growth, independence, and meaningful connections.

• Responsibilities: Support the development of daily living skills (ADLs) to foster greater independence. Promote active participation in community life through recreational and social activities. Encourage and support the formation of meaningful relationships with peers and community members. Assist in working toward and achieving personal goals. • Available
• Shifts (can be flexible): We are looking to fill weekend shifts, with the possibility of one weekday evening after 3pm. This is a part-time position for approximately 15 hours per week supporting this individual. The weekly schedule is semi-flexible, with additional flexibility available on weekends for extra hours. Additional part-time or full-time staffing opportunities within the agency may also be available.
• Pay
• Rate: $24 per hour when providing direct support. The hourly rate for indirect/training is $16 per hour.

The individual does not have the authority to set the staff salary in this model. The Mentor is paid at the provider’s rate for the service based on an assessment of the specific support needs of the individual in the areas of behavioral support, communication support, and physical support needs.
