Schedule

• Full-Time with weekend rotation

• What
• You’ll Do: Execute therapeutic care plans crafted by a licensed Physical Therapist to restore function and prevent decline Provide in-home treatments like range-of-motion, strengthening, balance training, and gait assistance Track progress and report changes using user-friendly EMR software Teach self-care techniques and guide caregivers to reinforce therapy between visits Actively collaborate with PTs, nurses, and other home health professionals for optimal outcomes

Qualifications
