DDRC is seeking a Licensed Practical Nurse (LPN) to provide nursing care and coordination for individuals in community-based settings. This role supports person-centered care, works closely with interdisciplinary teams, and ensures high-quality, compliant healthcare services outside of a traditional hospital environment. What You'll Do In this role, you'll provide nursing care and coordinate health services to support individuals in community-based settings. You'll partner with care teams, medical providers, and families to ensure quality care, accurate documentation, and regulatory compliance. Deliver nursing care, including assessments, medication coordination, and physician order implementation Coordinate care and communicate with providers, teams, and families Maintain accurate, compliant medical records Act as liaison with physicians, pharmacies, and other providers Monitor health changes and coordinate follow-up care Ensure timely completion of exams, consents, and orders Train and guide staff on medical procedures and infection control Support quality reviews and regulatory compliance Participate in on-call rotation Mentor and onboard new nursing staff Provide specialized support (e.g., G-tube care) Lead or assist with trainings, committees, and projects DDRC is an Equal Opportunity Employer and are committed to providing reasonable accommodations to qualified individuals with disabilities in accordance with the Americans with Disabilities Act (ADA).
