• Pro-actively manage chronic diseases (e.g., diabetes, hypertension) to prevent complications and improve overall quality of life.
• Optimize
• Pharmacotherapy: Perform comprehensive medication management, interpret labs, and safely adjust dosages under collaborative practice agreements.
