Locations
Provide personal care, mobility support, and vital signs monitoring to multiple patients daily in their homes or care facilities. Collaborate with the nursing team to follow care plans and report changes in patient conditions to the case manager.
Must hold a current Washington State CNA license, BLS certification from the American Heart Association, and a valid driver's license with a reliable vehicle. Previous experience working within a Home Health or Hospice agency is required.
