Coordinates pre- and post-transplant patient care, including evaluation testing, patient education, and multidisciplinary communication. Manages the transition of patients through referral, listing, inpatient rounding, and long-term follow-up care.
Requires a state clinician license (e.g., paramedic) with 4 years of transplant experience, or a nursing degree (Associate or Bachelor) with 2 to 4 years of transplant experience. Must hold a current BLS certification and a valid state or compact nursing license if applicable.
