Assess hospice patients to identify psychosocial, financial, and community needs while providing counseling and crisis intervention. Facilitate community resources and assist families with advance directives and funeral planning.
Requires a Bachelor's or Master's degree in social work from an accredited program and a current state license. Must have at least one year of clinical social work experience and a valid driver's license with auto insurance.

Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals. Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues. Facilitate community resources based on patient/family needs.
