Occupational Therapist Assistant PRN

As a Home Health COTA, you won’t just follow a plan—you’ll help bring it to life. Every day, you’ll step into patients’ homes and empower them to regain independence, rebuild confidence, and improve their quality of life in the environment that matters most to them.

This is a flexible PRN opportunity designed for clinicians who value autonomy, meaningful patient connections, and the ability to see the direct results of their work.

• What you’ll do
• Deliver hands-on occupational therapy interventions in patients’ homes under the guidance of a licensed Occupational Therapist
• Help patients regain independence with activities of daily living (ADLs) like dressing, bathing, and mobility
• Track progress and provide real-time feedback to adjust care for the best outcomes
• Educate patients and families on adaptive techniques and equipment that make everyday life easier
• Maintain accurate, timely documentation to ensure continuity of care
• Collaborate with a multidisciplinary team committed to exceptional patient outcomes

• What you bring
• Active Certified Occupational Therapy Assistant (COTA) license
• Current CPR certification
• Valid driver’s license & auto insurance
• Ability to work independently across different home environments
• Strong communication skills and a patient-first mindset

Preferred

• Home health or SNF experience
• Familiarity with HomeCare HomeBase (HCHB)

• Physical Demands
• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Must be able to lift, carry, pull, or push up to 100 lbs..
