Community Outreach Coordinator

Are you a dedicated and detail-oriented Community Outreach Coordinator looking to make a meaningful impact on patient care? PanCare of Florida is seeking a skilled and compassionate Community Outreach Coordinator to join our team, supporting our Ryan White program. In this role, you will be at the forefront of ensuring the safe and effective use of medications, working closely with healthcare professionals and patients to optimize treatment outcomes. • What You’ll Do
• Conduct outreach and education for individuals living with or at risk for HIV
• Provide Rapid HIV testing and ensure follow-up care for individuals testing positive
• Support individuals with counseling, linkage to care, and retention services
• Re-engage individuals who have missed appointments or fallen out of care
• Distribute health education materials and prevention resources
• Document testing, outreach, and program services accurately
• Build relationships with community organizations and healthcare partners
• Assist case managers with referrals, intake support, and client documentation
• Attend community events, health fairs, and outreach opportunities (some evenings/weekends may be required)

Preferred

• 2+years' experience in community outreach, social services, public health, or a related field required.
• Experience working with underserved populations or individuals affected by HIV
• Familiarity with the Ryan White Program, HIV care navigation, or case management (preferred but not required)

• Physical Requirements
• Ability to walk, stand, or sit for extended periods
• Ability to lift up to 25 lbs. (outreach materials)
• Local travel required to community locations and events

Hours: Monday through Friday from 8am to 5pm (Weekends and Evenings as required)
