The RN will assess consumer eligibility, establish and revise care plans, and authorize service packages for older adults and individuals with disabilities. Additionally, the role involves facilitating discharge activities and providing consultation to providers and program staff.
Candidates must hold a current Massachusetts RN License and have at least two years of clinical or community-based experience. Extensive knowledge of the needs of elders and people with disabilities is required.
