The Housing Specialist links residents to community programs and leads multi-disciplinary assessment teams to coordinate services. They are also responsible for facilitating care conferences and planning social, educational, and recreational activities for residents.
Candidates should have at least two years of experience in human services, preferably working with the elderly. A bachelor's degree in social work or a related field is preferred, along with proficiency in Microsoft Office.
