Must hold a current LPN/LVN license and CPR certification, with at least three years of nursing experience. A valid driver's license and access to a dependable vehicle are required for travel to patient homes.
Full Time or Part Time POSITION SUMMARY: The Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING: Develops and initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patients, families, and caregivers. Provides direct patient care as defined in state’s Nurse Practice Act. Implements plan of care initiated by the Registered Nurse. Provides accurate and timely documentation consistent with the plan of care. Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s). Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the Registered Nurse and/or Assistant Director of Nursing or Director of Nursing. Uses equipment and supplies effectively and efficiently. Participates in personal and professional growth and development. Performs other duties as assigned by the Registered Nurse. Other appropriate services and special projects as assigned. QUALIFICATIONS & REQUIREMENTS FOR THE POSITION: Current Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) licensure in state of practice. At least three (3) years of nursing experience in health care, home health experience preferred. Current CPR certification required. Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation. PREFERRED KNOWLEDGE AND SKILLS: Excellent time management, organization, and problem-solving skills. Must have outstanding verbal and written communication skills. Must be self-motivated and able to work independently while being collaborative and able to work as part of an interdisciplinary team. SUPERVISORY RESPONSIBILITIES: None. Significant portion (>50%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation. Work schedule may occasionally require evening, night, holiday, or weekend assignments.
