Physical Therapist (Home Health PT)

Choice Health at Home is seeking a compassionate and skilled Physical Therapist to provide in-home rehabilitative care. The Physical Therapist evaluates, develops, and implements individualized treatment plans to improve mobility, reduce pain, restore function, and enhance patients' quality of life.

Responsibilities

• Complete initial patient evaluations and develop individualized treatment plans.
• Provide physical therapy services in the patient's home.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on exercises, safety, and disease management.
• Maintain timely and accurate clinical documentation.
• Collaborate with physicians and interdisciplinary team members to ensure quality patient care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license.
• CPR certification required.
• Home health experience preferred.
• Homecare HomeBase experience preferred.
• Valid driver's license, reliable transportation, and auto insurance.
