The Case Manager provides time-limited services for functionally impaired seniors and adults with disabilities, including intake, assessment, and care plan coordination. They advocate for clients to secure housing and benefits while collaborating with community partners and mental health services.
Candidates must have a minimum of 2 years of experience working with older adults and knowledge of Alameda County community resources. Strong collaborative skills and experience with Adult Protective Services (APS) are highly valued.

$68,000 - $73,000 Annually (DOE) Benefits (subject to election and eligibility): Fully Paid Medical and Dental Plans; Health Reimbursement Card, Paid Time Off (personal, vacation, and sick time), 403B with company match. This position is union-represented by SEIU Local 1021 and is subject to the applicable collective bargaining agreement.
