Responsibilities

Provide comprehensive outpatient primary care services for patients of all ages. Diagnose and manage acute and chronic medical conditions. Perform annual wellness visits and preventive health screenings. Manage chronic diseases including diabetes, hypertension, COPD, and other common primary care conditions. Coordinate care with specialists and ancillary providers as needed. Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices. Maintain timely and accurate documentation within the electronic medical record system. Collaborate with physicians, advanced practice providers, nursing staff, and support personnel to ensure continuity of care.
