Responsibilities

Healthcare & Emergency Care: Provide direct healthcare and emergency care for people supported by Seven Hills Rhode Island, ensuring their health needs are met at home and in the community. Health Assessments: Complete initial and annual health assessments for participants, ensuring up-to-date care plans tailored to their specific needs. Health Education: Educate participants and staff about health maintenance, medication administration, and specialized procedures. Documentation & Records: Maintain confidential health records in accordance with regulations, ensuring all documentation is timely and accurate in the electronic health record system. Medication & Treatment: Train non-nursing personnel on proper medication and treatment administration, and oversee their compliance. Multidisciplinary Team Collaboration: Work with a team of healthcare professionals, families, and agencies (e.g., BHDDH) to ensure continuous care and support. On-Call Responsibilities: Participate in the Nurse on-call system to provide 24-hour telephonic coverage on a rotating schedule. Training & Support: Assist in the training and support of Direct Support Professionals (DSPs) and other team members on healthcare needs, procedures, and safety protocols. Medical Appointments: Provide transportation to medical appointments, if necessary, and assist with scheduling and follow-up care. Regulatory Compliance: Ensure all activities comply with the Behavioral Healthcare, Developmental Disabilities, and Hospitals (BHDDH) standards and regulations.

Qualifications

High school diploma or GED required. Licensed Practical Nurse (LPN) with current license in Rhode Island. Basic Life Support (BLS) certification required. 1-3 years of experience in nursing, particularly with adults with intellectual or developmental disabilities, preferred. Valid driver’s license with an acceptable driving record.
