Provide post-crisis stabilization and facilitate connections to ongoing behavioral health care for clients and their families. Responsibilities include conducting formal assessments, developing safety plans, and coordinating with internal and external stakeholders for effective discharge planning.
Requires a Bachelor's degree in Psychology, Counseling, Social Work, or a related field and must qualify as a Mental Health Professional (MHP) per Illinois Rule 132. Candidates must possess a valid driver's license and the ability to transport clients.

Responsibilities include conducting formal assessments, identifying treatment requirements, and developing comprehensive safety plans in collaboration with clients and their families. The role also involved working closely with other programs, such as Mobile Crisis Response (MCR) to ensure clients receive the necessary support and resources.
