Hampton, Virginia Overview Works under the supervision of the inpatient Clinical Manager to provide quality therapeutic services for patients and their families including individual, family, and group therapy, assessment, treatment planning, and documentation, in order to reduce or eliminate psychological, behavioral, or chemical dependency problems that necessitated admission.
Actively performs as a member of an interdisciplinary team and demonstrates the knowledge and skills necessary to provide clinical care appropriate to the age of the patient served.
As part of the interdisciplinary team, leads multi-disciplinary rounds and treatment teams to ensure patient treatment goals are identified and reviewed as part of discharge planning.
What you will do Accurately assesses biopsychosocial functioning of assigned patients and residents by using effective interviewing skills, behavioral observation, mental health assessment skills, and collateral sources of information.
Synthesizes information into a written document that provides valuable information for treatment planning.
Completes Diagnostic Summaries as assigned.
Provides quality and clinically sound individual, family, and group therapy interventions based on assessed needs that are appropriate for age, stage of development, and level of functioning.
Serves as the point of contact for family members and assures they are active participants in the treatment process with patient consent.
Provides education for families on suicidality, diagnosis, and plan of care as appropriate.
Addresses patient/family questions and issues.
Collaborates as an active member of the interdisciplinary team.
Communicates patient and family needs to team members, integrates clinical interventions with the plan of care, and evaluates progress on objectives.
Actively participates in team meetings.
Completes individualized, thorough medical record documentation according to RBHC policy.
Individual, family and group therapy progress notes completed on date that service is provided.
Maximizes use of clinical supervision and apprises Director of complex or problematic clinical issues.
Establishes and maintains solid relationships with referral sources, agency personnel, and community providers.
Takes on additional work and/or assists others in the completion of their work during periods of high census and heavy workloads.
Uses non-patient care time in productive ways to benefit the facility.
