Job description
The Dialysis RN (Registered Nurse) is responsible for providing care to patients who are in chronic and acute kidney failure who require dialysis or hemodialysis to filter out waste from their bodies. The Dialysis RN monitors patients during their dialysis session and works under the supervision of the nursing leader to coordinate the delivery of care to patients requiring dialysis.
As a registered nurse, the Dialysis RN is also responsible for assessing, planning, delivering, and evaluating each level of nursing care provided throughout the patient's stay and actively planning to manage patient outcomes across the continuum of care. Collaborates with the entire care team and communicates with family and patients.
RN Dialysis
Oversees the preparation and delivery of dialysis, including mandatory alarm tests and equipment tests are performed
Reviews physician orders, collects pre-treatment dialysis data and reviews patient records prior to dialysis
Conducts pre-dialysis patient assessment including, vital signs, assessing vascular access, lab findings and patient’s general health and medication history
Documents patient assessment findings, interventions, treatment results in the electronic medical records
Advises providers of any significant change in patient condition and shares any other information pertinent to the care of the patient
Discusses patient concerns and questions relevant to care
Oversees dialysis technicians in the performance of dialysis, monitors patient reaction to treatment and performance of dialysis machines and demonstrates an in-depth understanding of the mechanics of dialysis.
Oversees the disassembly and disinfection of dialysis equipment, surveils and tests the dialysis water to ensure machines are operating within the approved parameters
Demonstrates working knowledge of dialysis laboratory tests, medications, infection control.
Participates in Quality Assurance Performance Improvement (QAPI) plan.
Responds appropriate and provides assistance in emergency situations
Registered Nurse
Performs as RN for assigned patients with delivery of care based on acuity level, identified needs, and physician orders; Administers treatments and care effectively and skillfully; Demonstrates clinical competence in the delivery of patient services
Develops and maintains current patient treatment plan to include patient problems, goals, and interventions
Continually assesses patients’ physical, psychological, social, and educational needs to determine level and type of care, and evaluates the outcomes of interventions and implements necessary changes to care
Administers medications, IV therapy, and blood products within an acceptable error rate
Supervises care provided by LPN’s, and non-licensed assistive personnel in the provision of direct patient care; for assigned LPN, the RN will sign off on assessments performed by the LPN and is responsible for performing the patient admission assessments prior to handing off to the LPN
Assesses, collaborates, and coordinates patient’s discharge planning needs and readiness for discharge with other member of the care team
Documents assessments, interventions, and evaluations of care accurately, objectively & timely
Coordinates patient care with other departments/disciplines
Receives, transcribes, and carries out physician orders accurately and timely
Assesses patient/family learning needs, abilities, and readiness to learn and integrates needs into plan of care
Provides appropriate education/orientation to patients/families
Demonstrates safe and effective use of equipment
Demonstrates respect for patients/families
Serves as an advocate for patients/families regarding decisions affecting the POC
