The LPN is responsible for collecting resident data, implementing health plans of care, and documenting nursing interventions. They also collaborate with the household team to prioritize care and delegate tasks to Certified Nursing Assistants.
Candidates must be a graduate of an approved nursing school and hold a current LPN license in North Carolina. Current CPR certification and the physical ability to lift and position residents are required.

1. Participates in collection of data from the resident or other appropriate source 2. Recognizes the relationship of assessment findings as related to resident’s diagnosis 3. Records and reports the results of the nursing assessment. Determines immediate need for intervention.
1. Participates in identifying resident’s needs through suggestion of goas and interventions reviewed by RN.
1. Participates in implementing the resident’s health plan of care 1. Prioritize and perform interventions 2. Recognize responses to interventions 3. Modify immediate interventions based on changes in resident status
1. Reports and records resident’s response to nursing interventions 2. Suggests revisions to plan of care for review by RN
1. Responsible for accurate and timely documentation 2. Demonstrates effective communication skills with residents, representative of resident, physicians, and all members of the household team 3. Documents in accordance with facility policies and procedures using the electronic medical record and all other communication tools 4. Receives and transcribes physician orders according to policy 5. Gives and receives a complete report at the beginning and end of assignment: includes correct counting and documentation of controlled substance
1. Participates in teaching and counseling of residents and families 2. Teaching/Counseling uses established teaching plans and follows established policies and protocols
1. Participates in the Household to organize the work of the household and establish priorities. 2. Assign nursing care activities to other LPNs 3. Delegates resident care to verified Certified Nursing Assistants; retaining accountability for nursing care
1. Graduate of an approved school of nursing 2. Currently licensed as a LPN in good standing with the North Carolina Board of Nursing 3. Current CPR Certification 4. Previous experience with geriatric residents desirable 5. Good physical and mental health 1. Must be able to walk and stand for long periods of time 2. Must be able to assist with lifting, turning, and positioning residents 3. Must be able to push and manipulate equipment
