The Case Manager assesses and connects individuals and families to behavioral health and social services through person-centered planning. Responsibilities include resource coordination, advocacy, and maintaining compliant documentation in accordance with regulatory standards.
A Bachelor's degree in Human Services or a related field is required, with a preference for experience in behavioral health or substance use support. Candidates must possess a valid driver's license, a reliable vehicle, and pass required state background checks.
